Hacodefu/Hacodeo Scholarship Application Form
Those eligible to apply Highschool Scholarship are needy students who scored a minimum of 300 marks in KCPE for
FORM ONE applicants or a minimum of grade C in the previous year for current high school
students between FORM TWO and FORM FOUR.
Those eligible to apply College Scholarship are needy students who scored a minimum of grade C in KCSE.
N/B:
Attach copies of supporting documents (certificates, result slips, report cards etc.)
and send an email to info@hacodefu.org or hacodefu@gmail.com or give to a HACODEO member near you.

Those eligible are Institutions or groups applying for Funds towards programs that would support needy
populations and or the society in general.

- Beginning and ending dates of the project/program
- Geographic areas to be served
- Signatures and the program managers/directors
- Brief description of the program or project
- Outcomes you plan to achieve
- Who the project serves and why it is important
- Why your organization should receive the funds to implement the project and how the funds will be spent
- Organization information
- Mission statement
- Summary of your organization’s history
- Outline of current programs and activities
- Highlight accomplishments
- Purpose of the grant
- Identify the problem to be addressed and the needs to be met by the project
- Describe the goals and overall impact of the project or program
- Describe the program objectives, activities, strategies, staffing,
partners, timelines and explain how the design will enable you to address the problem or need
- Sustainability: Specify your plans for financing the project at the
termination of the grant. List other financing sources or strategies that you are developing.

- Interim and final evaluation and expenditure reports will be required for every grant awarded.

reference to long-range plans of the organization
